Access To Medical Records
You can register on-line to access your medical records. Please ask at reception for a form to complete your registration and allow 2-3 weeks for your request to be processed. You will be able to view your medical records from the date access is requested.
Alternatively copies of your own personal medical records are available on request. You can apply to access your medical records via our 'Access to Medical Records Form'. To ensure that the information is translated correctly, we insist that a healthcare professional is available to assist. This may be via an appointment. Please note a charge is made for this service to cover admin time and printing costs.
Confidentiality & Data Protection
The practice complies with Data Protection and Access to Medical Records legislation. Identifiable information about you will be shared with others in the following circumstances:
- To provide further medical treatment for you e.g. hospital referrals
- To help you get other services e.g. from the social work department. This requires your consent.
- When we have a duty to others e.g. in child protection cases.
Reception and administration staff require access to your medical records in order to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.
